JTCHS Flamingo Medical Center Achieved Special Recognition as an Age-Friendly Health System

Committed to Care Excellence

Jessie Trice Community Health System (JTCHS) is proud to announce that its Flamingo Medical Center, located in Hialeah, FL. has achieved special recognition as an Age-Friendly Health System —Committed to Care Excellence by the Institute for Healthcare Improvement (IHI), joining the ranks of organizations globally achieving success toward excellence in care for older adults.

This distinction recognizes health care organizations working toward the reliable practice of a set of four evidence-based elements of high-quality care for all older adults, known as the 4Ms: What Matters, Medication, Mentation, and Mobility. These elements focus on understanding patients’ individual priorities, ensuring appropriate medication use, addressing mental and cognitive health, and supporting safe mobility.

With this latest achievement, Flamingo Medical Center becomes the third JTCHS facility to receive this designation, joining the JTCHS Barbara Jordan Health & Wellness Center in Miami Gardens and JTCHS Fatima Zafar Health & Wellness Center in Liberty City.

“We are absolutey thrilled to have achieved this recognition, said Ryan R. Hawkins, president and CEO of JTCHS. “This milestone further demonstrates our ongoing commitment to providing high-quality, patient-centered care for all older adults we serve.”

“Our older adults deserve care that respects their individual needs and supports their health and independence,”added Dr. Joycelyn Lawrence, chief medical officer of JTCHS. “By focusing on what matters most to our patients, we can help them maintain their quality of life and receive the personalized care they deserve.”

Through its commitment to age-friendly care, JTCHS continues to prioritize approaches that address the changing health needs of older adults, helping them remain active, independent and engaged in their care.

Age-Friendly Health Systems is an initiative of The John A. Hartford Foundation and the Institute for Healthcare Improvement (IHI), in partnership with the American Hospital Association (AHA) and the Catholic Health Association of the US (CHA). For more information, visit www.ihi.org/agefriendly.
